To register a new patient you will need to live within our Practice Boundary. Once you have completed the form please call the surgery on 0208 515 1300 to book a registration appointment so that the registration can be completed.
Once you have made the appointment, you will need to come to the surgery with 1 proof of identity and 1 proof of residence to complete your registration.You must provide one example from the list below as proof if identity for anyone over 16 years of age. Alternatively if you prefer you can upload these then send them to us.
- Current Passport
- Medical card/Birth Certificate
- Immigration Documents
- National Identity Card
Also, you must provide 1 example from the list below as proof of Residency.
- Council Tax Bill
- Housing Association Rent Book/ Tenancy Agreement
- Current Utility Bill ( excluding Mobile Phone Bill)
- Current Bank Statement
Important Note: if you are registering any children under the age of 6, please bring all their immunisation records (i.e. Red Book/immunisation record), on the day of the registration.Also if you have been screened for TB, please bring the documentation.Without the above mentioned documentation, your registration will not be completed
Acceptance on to the list is as at the discretion of the Doctor and we will always ask you for proof of identity. Please note that your medical records may take several weeks to arrive at the Practice.
There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.
We have the right to refuse to register patients if they have a violent flag against their record.
Step 1: Complete online registration form
Our online registration form will collect your details, a brief outline of your medical history and record your preferences. This information may be added to your record and will allow us to begin to provide you with medical services.
Please complete one form for each family member.
Step 2: Upload scans of photo identity and proof of address
Please deliver to the practice copies of two forms of your identity.
One form of identity must contain a photo of yourself and the other must provide evidence of your current address. You may upload your forms online.
